ADMISSION INFORMATION
Admission
During your admission, the admitting clerk will ask your personal data, medical history and insurance coverage. You will be required to present an identification card and any referral or authorization you may have been asked to bring. An identification wrist band that you will need to wear during your hospitalization will be provided. This band enables the staff to coordinate the services you receive. Please wear this band at all times.
Watcher
Only one (1) watcher for the ward patients and two (2) watchers for private room patients is allowed. A watcher’s pass will be issued to you upon admission. Watchers’ pass shall be worn at all times.
Admission Kit
You will be issued an admission kit for your personal needs upon your admission. The kit consists of thermometer, measuring cup, bottle of alcohol, spoon & fork, soap, tooth brush, toothpaste, face towel, and slippers. A pillow will be given to you on admission which you can bring home when you are discharged. Additional pillows may be requested with a minimal cost.
Financial Information
Your Attending Physician may give you an estimate of probable expenses before your admission. A deposit will be requested from you at the time of admission in accordance with a schedule of rates set by administration. The deposit requirement is waived in emergency cases but we request that the appropriate deposit be made as soon as possible after the emergency.
The Billing Section provides patients with a Notice for Initial/Additional Deposits and a running bill every other day. We advise that you make arrangements to pay regularly in order that the bill does not accumulate.
